Born in Dublin, Gilmartin played for St Kevin's Boys Club in his hometown before accepting a professional contract with Walsall in 2005. He spent five years at the Bescot Stadium, including loan spells at Worcester City and Hednesford Town, and also made his international debut at under-19 level for the Republic of Ireland in February 2006, followed by the under-21s a year later. In all he made 37 first team appearances for Walsall, but in the summer of 2010, having turned down a renewed contract offer, the 6ft 5in Irishman made the step up to Championship side Watford. He played second fiddle for most of his two seasons at Vicarage Road, although he did have successful loan spells with Yeovil Town and Crawley Town, both in 2011-12.
After his contract with Watford was not renewed and following a glowing reference from Paul Wotton, a team-mate whilst at Yeovil, Gilmartin signed for Argyle in July 2012. A year later, after 16 appearances between the sticks, Gilmartin returned to Dublin to sign an 18-month deal with League of Ireland side St Patrick's Athletic.
After a total of ten appearances for St Pat's - just a single league game and nine cup outings - he returned to Watford in August 2014, signing a one-year deal as a back-up for Heurelho Gomes, two years after he first left Vicarage Road.
Gilmartin didn't make the first team in his second spell with Watford but did commentate on the club's away games for BBC Three Counties Radio, and took coaching qualifications which helped him take some training sessions at the club's academy. He was released, however, at the end of the 2016–17 season and that summer signed for League Two Colchester United as a player-coach.
In the summer of 2019, after 26 games in two seasons at Colchester, he rejected the offer of a new contract and moved to the Championship with Bristol City. Now 32, his main focus turned to coaching, with responsibility for the club's under-23 side, and just four months later he accepted the role of goalkeeping coach with the Republic of Ireland's under-21s.
In June 2021 Gilmartin became assistant manager at Swindon Town and six months later, when Swindon's manager became interim manager at Ipswich Town, Gilmartin joined the Tractor Boys' coaching team. He went on to become Ipswich's Head of Goalkeeping, with another ex-Pilgrim, Christian Walton, Ipswich's first-choice custodian.
